REFUND, DEPOSIT & CANCELLATION POLICY
Effective for all services provided by Farrahfied & Kai Vida Face Spa
Permanent Makeup & Cosmetic Tattoo Services
All permanent makeup and cosmetic tattoo services are non-refundable.
Permanent makeup is an elective cosmetic procedure. Results vary based on skin type, medical history, lifestyle, pigment retention, aftercare compliance, and individual healing response. Because of these variables, specific results cannot be guaranteed.
Prior to beginning any procedure, an extensive consultation and mapping process is performed to ensure mutual understanding of the desired shape, symmetry, and color. By proceeding with the service, you acknowledge and approve the design.
Additional sessions may be required to perfect and maintain results.
All services are performed in accordance with New Jersey sanitation and cosmetic tattooing regulations.
Deposits
A non-refundable deposit is required to secure all appointments for permanent makeup services.
• Deposits are applied toward the total service balance.
• Deposits are non-transferable.
• Deposits are non-refundable under any circumstances.
Your appointment time is reserved exclusively for you, and we decline other bookings for that time slot.
Rescheduling Policy
We require a minimum of 48 hours’ notice to reschedule an appointment.
• With proper notice, your deposit may be transferred one time.
• Failure to provide 48 hours’ notice results in forfeiture of your deposit.
• Same-day cancellations may be charged up to 100% of the service fee.
Late Arrivals
Clients arriving more than 15 minutes late may have their appointment:
• Shortened
• Rescheduled
• Canceled with deposit forfeited
No-Show Policy
Failure to appear without notice will result in:
• Immediate forfeiture of deposit
• Requirement of a new deposit to rebook
• Possible refusal of future appointments at our discretion
Touch-Up Policy & Expiration
Permanent makeup procedures require a follow-up session to refine and perfect pigment retention.
• The initial touch-up must be completed within 6–8 weeks of the original procedure date.
• Touch-ups after 8 weeks are considered a color boost or new session and priced accordingly.
• Failure to schedule within the 6–8 week window voids any included touch-up pricing.
• It is the client’s responsibility to book their follow-up appointment.
We are not responsible for premature fading due to:
• Sun exposure
• Use of retinols, acids, or resurfacing products
• Medical conditions or medications
• Failure to follow aftercare instructions
• External procedures (laser, chemical peels, microneedling, etc.)
Medical Disclosure & Eligibility
Clients must disclose all medical conditions, medications, allergies, and previous cosmetic procedures.
Failure to disclose relevant medical information that affects healing or pigment retention releases Farrahfied & Kai Vida Face Spa from liability related to results or complications.
We reserve the right to refuse service if a client is not an appropriate candidate under New Jersey health and safety standards.
Liability Acknowledgment
By booking and receiving services, you acknowledge that:
• You understand the nature of permanent makeup procedures
• Results vary and are not guaranteed
• Touch-ups may be required
• You agree to follow all aftercare instructions
• You release the business and its technicians from liability related to healing variations, pigment retention, color changes, or fading
New Jersey Sales Tax
All services and applicable retail products are subject to New Jersey state sales tax where required by law.
• New Jersey sales tax (currently 6.625%) will be added to the total service amount at checkout.
• Advertised prices do not include sales tax unless explicitly stated.
• Sales tax rates are subject to change in accordance with state law.
Clients are responsible for payment of all applicable taxes at the time services are rendered.
Chargeback & Payment Protection Policy
By booking and receiving services at Farrahfied & Kai Vida Face Spa, you agree:
• All services are custom, time-based, and rendered in full at the appointment.
• You authorize payment for services performed.
• You will not initiate a chargeback or payment dispute for services rendered.
In the event of a dispute or chargeback, the following documentation may be submitted as evidence:
• Signed consent forms
• Intake forms
• Pre-procedure approval documentation
• Before-and-after photos
• Booking confirmations
• Communication records
Fraudulent chargebacks will be pursued to the fullest extent permitted by law and may result in collections action and permanent refusal of future services.
